Grant Submissions

The Lindorf Family Foundation meets March, September, and if needed, December annually.

Our next meeting date is set for Tuesday February 25th, 2020 2:00pm.

Grants in Excess of $10,000

Completed grant applications may be submitted for review at any time. The Trustees of the Foundation will evaluate these applications at a regularly scheduled meeting after submission. Every effort will be made to respond to applications in a timely way.

All requests for funding from the Lindorf Family Foundation should include the following information or equivalent information, as appropriate.

I. COVERSHEET

  • Date

  • Name and address of organization

  • Names and titles of trustees, officers and executives

  • Telephone numbers of contact person and for the organization’s main office

  • A short statement summarizing the purpose and activities of the organization

  • A short statement summarizing the proposal that demonstrates its relationship to the organization’s mission.

  • The total cost of the project and the amount requested from the Foundation

II. NARRATIVE

  • Brief summary of organization’s history, including mission and goals,

  • Short description of current programs, activities, accomplishments, including the number of persons in Licking County and outside Licking County who receive benefits or services,

  • Statement of needs/problems to be addressed with grant funds; description of target population and benefit to the target population

  • Implementation schedule, length of time for project, long- term strategies for funding this project after funding from the Lindorf Family Foundation is expended.

  • List of other organizations, if any, participating in the project and their role,

  • List of other organizations from which funding has been requested for this project, including amount requested and response.

  • How an evaluation of the project will be conducted and a timeline for delivering the results to the Foundation.

III. ATTACHMENTS

  • Most recent annual financial statement, most recent 990 tax return, and any interim report, if available

  • Organization’s previous and current annual operating budget

  • Project budget (must include sources and uses of all funds for the project, not just funds from this source)

  • Copy of current IRS determination letter confirming status as a 501(c)(3) organization and your tax identification number

  • Any additional supporting documentation relevant to the organizations programs and in support of the application.

Submit five (5) copies of the completed application by United States Mail or one (1) copy by e-mail, including all attachments, to:

Grants under $10,000 do not need to prepare a presentation. All that we require is on the form.
